You will need the following information:

  • Completed W-9 form (upload as a PDF)

  • Small/Women/Minority-Owned business certification (upload as a pdf) - if applicable

  • Voided check (upload a scanned image)

  • Bank routing and account numbers

  • Federal Tax ID or Social Security Number

  • Legal Business Name

  • Contact Information (name, address, email, phone)

  • Account Administrator (person responsible for updates)

  • Commodity codes/descriptions of your products and services.
